  • Scientific Description
    Proteinase 3 (PR3) is a serine protease predominantly stored in the azurophilic granules of neutrophils and monocytes, where it contributes to the degradation of extracellular matrix proteins, pathogen destruction, and regulation of inflammation. Beyond its enzymatic activity, PR3 is notable as the primary autoantigen in granulomatosis with polyangiitis (GPA, formerly Wegener’s granulomatosis), a severe autoimmune small-vessel vasculitis. In GPA, autoantibodies known as PR3-ANCA (anti-neutrophil cytoplasmic antibodies) target PR3, leading to inappropriate activation of neutrophils, which then release inflammatory mediators and cause vascular injury and granulomatous inflammation in organs such as the respiratory tract and kidneys. PR3 is also expressed on the surface of neutrophils, especially during activation and apoptosis, where it can modulate immune responses and influence microvesicle production, further amplifying inflammation. Clinically, detection of PR3-ANCA is a key diagnostic marker for GPA and helps differentiate it from other vasculitides. Research into PR3’s structure and membrane interactions is ongoing, with the aim of developing targeted therapies for autoimmune vasculitis and understanding PR3’s broader roles in immunity and inflammation.
